Cindy Hasz is a geriatric care manager and patient advocate with over 25 years experience in all phases of elder care. From hospital to rural home care, she brings a special level of creativity, dedication and love to those she serves. One of her greatest strengths is team building and Grace Care Management reflects this with a highly professional medical staff of public health and registered nurses, licensed nurses, medical assistants and a broader multidisciplinary team of affiliated social workers. Cindy founded Grace Care as a way to support elder and family centered care, and to champion quality of life in all care settings.
